Program spotlight: Whole Person Wellness

In 2018, PATH launched a critical program to San Diego County— Whole Person

Rhonda with her clinical case manager, Kristina.Wellness. The program serves our neighbors on the street without a home who need access to healthcare services because of a physical health, mental health, or substance use issue.

Whole Person Wellness improves the well-being of individuals by connecting them to services that improve health and wellness—with the ultimate goal of helping them move into a permanent home.

In partnership with the local community and people like you, PATH works to end homelessness for individuals, families, and communities. Every dollar helps fund critical programs like Whole Person Wellness.

For every $1,000 we raise, PATH can help an individual like Rhonda take the final steps into a home of their own. You can help our homeless neighbors Make It Home this holiday season!
